Coast Guard
Active duty and reserve members are eligible to request a Coast Guard Institute (CGI) transcript which may be submitted for evaluation of possible transfer credit toward APUS degree programs. Comprehensive information regarding voluntary education, resources for members, and all the necessary forms are available online at http://www.uscg.mil/hq/cgi/ve/ve.asp.
Below is a summary of the necessary actions students must take to have an official CGI transcript sent to APUS for transfer credit evaluation. For additional information or assistance with these processes, students can visit the indicated Coast Guard websites and/or contact their local Education Services Officer (ESO).
1. Students must complete a request for Assessment, using form CGI-1561, Application for Voluntary Education Services.
The assessment will form the basis for the official CGI transcript and should be completed prior to requesting an official transcript. This form cannot be used to request official transcripts. For additional information on requesting an assessment, visit http://www.uscg.mil/hq/cgi/ve/assessment.asp. Specific instructions for completing the application can be found on page three of the document. Students will need to:
- Download and complete the Application for Voluntary for Voluntary Education Services.
- Attach all required documentation; see instructions for details on both required and suggested documentation.
- Sign the application and submit it to their local ESO.
Once submitted, the ESO will review the application as well as all attached documentation for completeness and correctness, then sign the application. It is preferred that ESOs submit completed applications via TACCTS, however if returned to students, completed applications may be submitted by mail to:

2. Students must request an Official Transcript, using form CGI-1564, Official USCG Transcript Request.
Once the assessment has been completed, students may request an official CGI transcript be sent to our offices for transfer credit evaluation. For additional information on requesting a transcript, visit http://www.uscg.mil/hq/cgi/ve/official_transcript.asp. Students will need to:
- Download and complete the Official USCG Transcript Request.
- Provide the APUS mailing address in the "Mail Official Transcript to" area of the form:
