Coast Guard

Active duty and reserve members are eligible to request a Coast Guard Institute transcript.  Information and the necessary forms are available online at www.uscg.mil/hq/cgi/credit.

1.  Complete form CGI 1560/04e (Educational Assessment Worksheet/EAW)

Students must complete the worksheet and attach documentation to validate that the American Council on Education has evaluated the course(s) for college credit.  Electronic signatures are not accepted.

Provide the following documentation:

  • Course completion letters, certificates, or graduation letters from the school 
     
  • People Soft History of training and career 
     
  • Student copy of college transcripts (for personalized degree planning purposes) 
     
  • Service-wide exam profile letter (for PNA credit) 

Note:  The following should not be provided: 
  • Completion letters for Coast Guard end-of-course exams  
     
  • Certificates of advancement

2.  Send the EAW, documentation and USCG Transcript Request form to the Commanding Officer at:
